Transition strategies handbook

'Regardless of the transition point, students, parents, and carers, and educators need to work together to manage and facilitate these transitions. This will ensure that there is a meaningful relationship between all stakeholders with the shared goal of maximising learning outcomes for the student. Within this supportive and positive environment, student wellbeing and learning is enhanced.'

Transition provided the catalyst to reflect on and consider family engagement.

The Ready Set Connect cluster has worked together to develop and grow transition practices within our schools that strengthen the relationships with parents and community. Through working as a cluster, we have had the opportunity to trial and implement evidence-based practices that have been co-created by schools, around transition and parent and community engagement. These strategies aim to support students to have reduced anxiety when transitioning to and from different school settings and ensure families do not become isolated, promoting a sense of belonging and social connectedness within the schools and the community as a whole.

The following transition strategies target specific student, family and school challenges, identified by the cluster. They include programs to support change, strategies focused on building relationships, strategies that are personalised to specific target groups, structures for open days and orientations, workshops for parents and student and communication methods.
